All Streptococcus bacteria look alike under the microscope. They are also hard to distinguish in microbiological cultures. Thus, it is almost impossible to tell them apart by looking at them, and instead, metabolic tests must be used to distinguish the various bacteria of the Streptococcus group. This is unlike the two groups of Staphylococcus,which have different colors in culture. Scientists believe that there are at least 40 confirmed types of Streptococcus, each having a few different strains. Scientists divide the 40 or more different types into 2 groups called clusters. Initially, the degree of hemolysins produced by the bacteria defined the clusters. Some Streptococcus do not produce any hemolysins and thus are nonhemolytic. Others produce either alpha or beta hemolysis. A beta-hemolytic Streptococcus produces the most severe effects in the body. A later method, called Lancefield grouping, categorized the types of Streptococcus based on immunological testing called serotyping. A particular Lancefield group refers to a protein antigen found on the surface of the bacteria. A combination of characteristics, including where the bacteria appear, presently determines the clusters. The clusters are the anginosus group, the bovis group, the mitis group, the mutans group, the pyogenic group, and the salivarius group. Streptococcus anginosus and a host of relatives make up the anginosus group. These alpha-hemolytic bacteria are commoninhabitants of the mouth and nose and do not cause disease. They are part of the A, F, and G Lancefield groups The bovis group consists of an intestinal bacterium called Streptococcus bovis. As its name implies, scientists isolated it from the intestinal tract of cattle. These Streptococci are alpha- or nonhemolytic and related to other Streptococcus bacteria specific to particular animals. Most cause no harm in their host animals. However, they can infect wounds and cause septic diseases. They have a D Lancefield grouping. The mitis group contains a diversity of alpha-hemolytic bacteria in the H and O Lancefield groups. Most live in the respiratory systems of humans and animals without causing disease. These bacteria can cause some types of pneumonia. Streptococcus pneumoniae, which causes human respiratory diseases, is a pathogen in the mitis group. The mutans group has no hemolysins and does not belong to a designated Lancefield group. They live in the mouths of many animals, where they glue themselves to the teeth. Many of them are involved in tooth decay and possibly gum disease. Streptococcus salivarius appropriately appears in the salivarius group. Like the mutans-group bacteria, they live in the mouth and do not produce hemolysin. It belongs to the K Lancefield group. The pyogenes group contains the bacterium that causes StrepTSS. These bacteria are beta-hemolytic and are highly capable of causing severe reactions. They are part of the A, B, and C Lancefield groups. Included in the pyogenes group are Streptococcus algalactiae, Streptococcus equisimilis, and Streptococcus pyogenes. Streptococcus algalactiae is a contagious bacterium ofmammals. It commonly causes ear, sinus, and throat infections of domesticated animals and humans. People who work with animals are likely to contract infections from this bacterium. In the 1960s, Streptococcus algalactiae became a leading cause of disease in human babies.Streptococcus equisimilis has several strains found in many mammals. They cause diseases similar to toxic shock syndrome in animals and humans. The bacteria spread thoughout the body causing high fever and damage to internal organs. Streptococcus pyogenes is the culprit that produces StrepTSS and other diseases in humans. Researchers also called it Streptococcus hemolyticus, Streptococcus erysipelatos, Streptococcus scarlatinae, and Micrococcus scarlatinae until they confirmed that all were the same bacterium. Their small size and the high variability of traits make bacteria difficult to identify.

The term trench fever refers to the crowded conditions in which troops fought in during World War I and World War II. Because the causative bacteria are passed among humans through contact with body lice, overcrowding, and conditions which interfere with good hygiene (including regular washing of clothing) soldiers were predispose to this disease. Currently, homeless people in the United States are sometimes diagnosed with this illness. The bacteria are sometimes passed through the bite of an infected tick.

The first clinical description occurred during World War I (WWI), but the condition has probably caused human infection for centuries. Trench fever was considered the most prevalent disease among Allied troops serving in the trenches during WWI. After WWI, trench fever became dormant, until it reemerged as an epidemic on the eastern European front during World War II. Since WWII, classic trench fever has almost disappeared as a clinical entity.Trench Fever attacked all armies and until the final year of the war baffled doctors and researchers. Chief symptoms of the disease were headaches, skin rashes, inflamed eyes and leg pains.Despite such wide-ranging symptoms the condition was not itself particularly serious, with patients recovering after some five or six days although prolonged hospitalisation amounting to several weeks was common.Trench fever is a louse-borne disease. The lice do not become infectious at once after feeding on a trench-fever patient; there is a latent period of some 8-12 days before they are dangerous to other people. Thereafter the excreta of the lice, rather than their bites, are infective. If these infective excreta be rubbed into a scratch or scarification trench fever develops in about eight days. The importance of this discovery about the excreta lies in the fact that persons may contract the condition who have never had lice upon them. The excreta is a dry powder, easily blown about, and so apt to reach the clothes. It remains infective for long periods and even when exposed to sunlight. Water on the other hand seems to diminish its infectivity quickly. The blood of trench-fever patients is infective to other patients when injected into their veins. Thus the parasite circulates in the blood. The parasite is also in the louse excreta. Cause of trench fever:The cause of trench fever is Bartonella quintana (also called Rochalimaea quintana), an unusual rickettsial organism that multiplies in the gut of the body louse. Transmission of the rickettsia to people can occur by rubbing infected louse feces into abraded (scuffed) skin or into the conjunctivae (whites of the eyes). The vector for Trench Fever was, of course the body louse, pediculus corporis which became infected by feeding on the blood of infected soldiers; spread was by migration of the louse and infection of the new host by the insect bite or by scratching the skin which was contaminated by the louse excreta. The excreta remained infective for long periods, weeks or months.The disease is classically a 5-day fever. The onset of symptoms is sudden with high fever, severe headache, back pain and leg pain and a fleeting rash. Recovery takes a month or more. Relapses are common.

It is now known that cat scratch disease (CSD) is a multisystem disease caused by a small Gram negative bacillus named Bartonella henselae, formerly known as Rochalimaea henselae. As early as 1932, physicians in the United States recognized patients with CSD, but the cause of the disease eluded detection until 1983 when researchers at the Armed Forces Institute of Pathology at Walter Reed Army Medical Center in Washington, D.C. detected the organism in lymph node tissue from patients with clinical CSD. Identification of the causative organism has allowed researchers to identify the epidemiology and transmission of the disease, identify atypical forms of CSD, develop serologic diagnostic tests, and perform placebo-controlled treatment trials.

A four year old girl presents to the pediatric clinic with a chief complaint of a slowly enlarging mass in her right armpit for the past two weeks. She has had no fever, but has had some loss of appetite. The mass was initially small and did not hurt; however, it has now grown to the size of an orange and has become painful. She cannot lower her arm due to the pain from the mass, and she carries her arm extended at 90 degrees to her body.

Movement of her arm exacerbates the pain. She does have a 3 month old kitten at home that she “rescued from a sewer”. It was covered with fleas when she found it. It playfully bites and scratches her. Three weeks ago it scratched her right thumb. The scratch healed, but a small “wart” has developed in the line of the scratch. There is a scab on the wart and her mother has tried to squeeze the wart, but no pus has come out.

ROS: Non-contributory. No cough. No weight loss. No change in activity other than being unable to use her right arm well.Exam: VS T 37.2, RR 18, P 102, BP 100/60. She is alert and very cooperative. HEENT is negative. Neck is supple without adenopathy. Chest is clear. Heart regular without murmur. Abdomen exam is normal without hepatosplenomegaly. Neurologic exam is normal (mental status, gait, strength, and reflexes). Her right axilla reveals an 8×8 cm firm, tender, mobile, warm, non-erythematous, non-fluctuant mass that is consistent with an enlarged axillary lymph node. Her right thumb has a 1 cm linear, non-inflamed, healing scar that is consistent with a kitten scratch. In the middle of the linear scar, there is a 3 mm brownish-red papule with a small central crust.

Lab: CBC WBC 8.0, 62% segs, 10% bands. Ultrasonography of the mass reveals that it is a matted group of about 5 lymph nodes which are mostly solid in appearance. There is evidence of a small amount of necrosis at the periphery of one of the lymph nodes.

Impression: Lymphadenopathy due to cat scratch disease.

Clinical course: Because the axillary node is enlarged and painful, you elect to treat her with oral azithromycin at a dose of 10 mg/kg/day for the first day and 5 mg/kg/day for the next 4 days. Serology for Bartonella henselae is obtained, and the result returns one week later with an IgG of 1:512 (a positive result is a value greater than 1:64). The node remains the same size for a week and then begins to get smaller. The adenopathy resolves in one month.
